Self-Guided vs Clinician-Guided Online Cognitive Behavioral Therapy for Atopic Dermatitis

A single-blind randomized noninferiority trial in adults with atopic dermatitis compared a 12-week self-guided online CBT program with a 12-week clinician-guided online CBT program, assessing change in POEM from baseline to postintervention and to 12-week follow-up.
Key findings
Postintervention, POEM improved 4.60 points (95% CI, 2.57-6.64) with self-guided CBT and 4.20 points (95% CI, 1.94-6.05) with clinician-guided CBT; the estimated mean difference in change was 0.36 points (1-sided 97.5% CI, -∞ to 1.75), below the 3-point noninferiority margin. No serious adverse events were reported; 151/168 (90.0%) completed postintervention, and clinician time averaged 36.0 minutes for guidance plus 14.0 minutes for assessments in the clinician-guided group vs 15.8 minutes for assessments in the self-guided group.
Study limitations
Single-blind design at a single center. The sample was predominantly female (84.5%).
Clinical implications
For adults with atopic dermatitis, a brief self-guided online CBT program was noninferior to clinician-guided CBT on POEM and required minimal clinician time. It offers a scalable option where psychological support resources are limited.
