This retrospective French national cohort study described the clinical features and outcomes of pediatric patients with both inflammatory bowel disease (IBD) and chronic nonbacterial osteomyelitis (CNO), enrolled from gastroenterological and rheumatological units between 2000 and 2024, and estimated the incidence of IBD-CNO among pediatric IBD patients.
37 pediatric patients were identified (median age 13.5 years); Crohn's disease was most common (25/37). At median 3-year follow-up, 75.7% achieved IBD-CNO remission, with anti-TNF-α therapy associated with the highest remission rate (82.1%, p=0.041). Incidence was estimated at 1 case per 20,000 patient-years among French pediatric IBD.
Retrospective design with potential underreporting due to low clinical awareness; reliance on voluntary case reports from participating units may miss cases; incidence estimate depends on EPIMAD regional cohort data extrapolated nationally.
Order whole-body MRI in IBD children with chronic bone pain to screen for CNO. Anti-TNF-α monotherapy appears effective for both conditions and may prevent bone complications; multidisciplinary involvement of rheumatology and dermatology is recommended.
