<jats:p>We report promising clinical outcomes using ustekinumab as a first-line therapy in bullous pemphigoid (BP) when combined with a short, 8-week regimen of super-potent topical corticosteroids. Furthermore, the CD40L/IL-4 ratio emerged as a potential predictive biomarker of treatment response, possibly reflecting a Th1-skewed immune environment that favors ustekinumab’s mechanism of action.</jats:p>
