Multiple Onychopapillomas and BAP1 Tumor Predisposition Syndrome

What was studied

A prospective single-center cohort of 47 known carriers of pathogenic BAP1 germline variants (enrolled Oct 10, 2023–Mar 15, 2024) was evaluated to determine the prevalence and histopathologic features of nail abnormalities.

Key findings

Nail abnormalities were present in 41/47 (87.2%); clinical findings consistent with onychopapilloma occurred in 39/47 (83.0%), including 35/40 aged ≥30 years (87.5%). Polydactylous involvement was seen in 38/39 (97.4%), and nail bed biopsy confirmed onychopapilloma in 5/5.

Study limitations

Single-center cohort of known BAP1 carriers without a control group; small sample (n=47); histopathologic confirmation was performed in only 5 patients.

Clinical implications

Multiple onychopapillomas across several nails are common in adults with BAP1 TPDS and may be a practical clinical clue to identify the syndrome in at-risk patients and families.
