This study evaluated bimekizumab (dual IL-17A/IL-17F inhibitor, 160 mg SC every 4 weeks) vs. placebo on MRI inflammation/structural lesions of the sacroiliac joint and spine, and radiographic progression, in adults with nr-axSpA (BE MOBILE 1) and r-axSpA (BE MOBILE 2) over 104 weeks, including a 52-week open-label extension.
By Week 52, bimekizumab reduced SIJ inflammation (SPARCC SIJ mean CfB: −7.6 nr-axSpA, −5.1 r-axSpA), spine inflammation (Berlin spine CfB: −0.5 nr-axSpA, −2.5 r-axSpA), and SIJ erosion scores (−1.8 both subtypes), with increased backfill and fat lesion scores; at Week 104, radiographic progression was minimal (mNY CfB: 0.03 in nr-axSpA; mSASSS CfB: 0.3 in r-axSpA).
- Observed case data only (no imputation), which may overestimate benefit in completers. - No direct comparator arm at Week 104 (all patients received bimekizumab after Week 16). - MRI sub-studies represent a subset of the full trial populations, limiting generalizability.
Bimekizumab produces meaningful reductions in SIJ and spinal MRI inflammation and erosion in both nr-axSpA and r-axSpA, with very little radiographic progression at 2 years—supporting its use as a disease-modifying option across the axSpA spectrum.
